Nöroloji servisinde yatan geriatrik hastalarda hastane ortamının uyku kalitesine etkisi

Abstract

Bu çalışma, nöroloji servisinde yatan geriatrik hastalarda hastane ortamının uyku kalitesine etkisinin belirlenmesi amacıyla yapılmıştır. Çalışmanın evrenini, Nöroloji Kliniğinde yatmış 194 hasta oluşturmuştur. Çalışmanın örneklemini, evreni bilinen örnekleme yöntemine göre 136 hasta oluşturmuştur. Veriler, anket formu, Pittsburg Uyku Kalitesi Ölçeği ve uyku düzenini etkileyen etmenler formu kullanılarak toplanmıştır. Araştırma öncesinde, etik kurul onayı, kurum izni ve hastalardan yazılı/sözlü onam alınmıştır. Verilerin değerlendirilmesinde yüzde, ortalama, Kruskal Wallis ve Mann Whitney U testinden yararlanılmıştır. Anlamlılık düzeyi p<0.05 kabul edilmiştir. Çalışma sonucuna göre Pittsburg Uyku Kalitesi İndeksi toplam puan ortalaması 5.25±3.057 olan hastaların alt bileşenlerden öznel uyku kalitesi puan ortalaması 1.09±1.000, uyku süresi puan ortalaması 1.00±0.644, alışılmış uyku etkinliği puan ortalaması 0.80±0.893, uyku bozukluğu puan ortalaması 1.01±0.590, uyku ilacı kullanımı puan ortalaması 0.17±0.414, gündüz işlev bozukluğu puan ortalaması 0.04±0.189 olarak bulunmuştur. Hastaların %28.7'si ışıkdan, %30'u odanın kalabalık olmasından, %30'u odaya sık girilip çıkılmasından, %24.3'ü uyku saatinde yapılan girişimlerden, %55.1'i ağrıdan, %25.7'si vücuda takılı tıbbi cihazlardan, %31.6'sı çevredeki gürültülerden etkilendiğini belirtmiştir Sonuç olarak, Hastanenin fiziki koşulları hastaların uyku düzenini etkilediği söylenebilir. Çalışmadan elde edilen sonuçlara göre uykusuzluğa neden olan etmenlere yönelik öneriler getirilmiştir.

In this study, the effect of hospital environment on the sleep quality of geriatric patients are analyse. In the first part of the study, a conceptual framework related to sleep has been established. In the second part, such factors as old age, parameters affecting sleep quality in old age, sleep disorders seen in neurological patients and the connection of nursing process with sleep are discussed in detail. In the third part of the thesis, a case study is conducted to understand sleep quality in geriatric patients. Accordingly, 136 geriatric patients who were hospitalized in Neurology Clinic of Yıldırım Beyazıt University Yenimahalle Training and Research Hospital in Ankara were investigated. Then collected data were analyzed statistically. In this context, the findings indicates that the mean score of subjective sleep quality among the subcomponents of the patients with a total score of 5.25±3,057 in the Pittsburg Sleep Quality Index is 1.09 ± 1.000, the average sleep time score is 1.00±0.644, the mean score of efficacy is 0,80±0,893, the mean score of sleep disorder is 1.01±0.590, the mean score of sleep medication use is 0.17±0.414 and the mean score of daytime dysfunction is 0.04±0.189. Besides, the patients stated that they are affected by light (28,7% of patients), crowdedness of the room (30% of patients), getting into and getting out of the room frequently (30% of patients), interventions during the sleeping hours (24,3% of patients), pain (55% of patients), medical devices attached to the body and noise in the environment (31,6 % of patients). In conclusion, it can be argued that physical conditions of the hospital have affected sleep pattern of patients. In accordance with the findings of the study, some proposals are also put forward for factors which trigger insomnia.
