Hastanelerin performansının veri zarflama analizi/analitik hiyerarşi prosesi bütünleşik yöntemi kullanılarak değerlendirilmesi

Abstract

AHP (Analitik Hiyerarşi Prosesi) ve VZA (Veri Zarflama Analizi) yöntemlerinin bütünleşik olarak kullanımını konu alan bu tez çalışmasının amacı, Ankara ilinde faaliyet gösteren Sağlık Bakanlığı'na bağlı kamu hastanelerinin göreceli etkinliklerinin ölçülmesi ve etkin /etkin olmayan hastanelerin belirlenmesidir. Çalışmada hastanelerin sağlık hizmeti üretmek için kullandığı 4 girdi ile ürettiği 5 çıktı belirlenmiş ve bu girdi ve çıktılarla iki farklı VZA modeli oluşturulmuştur. Bunlardan biri ağırlık kısıtlamasız VZA modeli, diğeri ağırlık kısıtlamalı VZA modelidir. Ağırlık kısıtlamasız VZA modelinde, AHP yöntemi çalışmaya dâhil edilmeden hastanelerin etkinlik analizi VZA ile yapılmıştır. Daha sonra girdi ve çıktılar hastane üst düzey yöneticileri tarafından karşılaştırılmış ve önemine göre ağırlıklandırılmıştır. Diğer VZA modelinde ise ağırlıklandırılmış girdi ve çıktılar ile hastanelerin etkinlik analizi yapılmıştır. Çalışma kapsamında Ankara'da faaliyet gösteren 26 kamu hastanesi değerlendirilmiştir. Bu hastanelerin sağlık hizmetlerini üretirken kullandıkları kaynakları tanımlanmış ve bu kaynakları ne derece optimum kullandıkları belirlenmiştir. Bunun için Efficiency Measurement System (EMS) 1.3.0 paket programı kullanılarak oluşturulan iki modelin VZA etkinlik ölçümleri elde edilmiş, analiz sonuçları yorumlanmıştır. Ağırlık kısıtlamasız VZA modelinde 13 hastanenin etkin olduğu belirlenmiş, ağırlık kısıtlamalı VZA modelinde ise ağırlık kısıtlamasız VZA modelinde etkin bulunan 3 hastanenin, ikinci model olan ağırlık kısıtlamalı VZA' da etkin olmayan modeller arasına girdiği görülmüştür. Çalışma sonucunda, ülkenin sağlık politikasını belirleyen karar vericilere ve hastane idarecilerine, kaynakların daha rasyonel bir şekilde kullanılması için yapılması gereken iyileştirmeler ve alınması gereken tedbirler sunulmuştur.

The aim of this study, which is about the integrated usage of the Data Envelopment Analysis (DEA) and the Analytic Hierarchy Process (AHP) methods, is to measure the relative efficiency of the public hospitals serve under the Ministry of Health and to determine the efficient/inefficient hospitals. 26 public hospitals in Ankara were included in this study. 4 inputs the hospitals use and 5 outputs they generate were specified while providing health service and two different DEA models were established using these same inputs and outputs. The first DEA model uses the inputs and outputs without weights and the second DEA model uses the inputs outputs with the weights derived from AHP. Each models were run separately and efficiency measurement was made. As a result of running the first DEA model, 13 hospitals were found to be efficient and this was 10 for the second model. In other words, it was seen that 3 hospitals that were efficient in the first DEA model became inefficient in the second model. In addition, the average efficiency score was 91% in the first state and 86% in the second state. Efficient and inefficient hospitals could use these efficiency scores in their future decisions. It is expected that efficient hospitals will maintain their position and inefficent hospitals will take the efficient hospitals in their reference sets as an example to become efficient. It is hoped that the results of this study can be used as leading principles by decision makers of health policy, and by hospital managers and staff. Finally, constraints of this study and the research areas are pointed out for future research on this subject.
